Job Description
We are looking for a Nurse Practitioner to support our interdisciplinary team (IDG) by complementing physician care, evaluating hospice eligibility, performing Face-to-Face visits, and ensuring patients receive proper symptom management.
In addition to CMS eligibility requirements, the Nurse Practitioner will also conduct patient visits for those under the care of our Medical Director, when patients choose him as the attending physician.
Some of the qualifications we're seeking include the candidate being an accredited NP, holding a current MI license, having experience with end-of-life care, and possessing strong communication skills.
