OVERVIEW:

Provide quality incident investigations for all allegations within the agency. Ensures that appropriate recommendations are made to safeguard individuals and provide the highest quality of services. Provides education and training to all staff in the areas of abuse prevention, incident reporting, positive approaches and other classes as identified. Ensures compliance with all applicable standards.

RESPONSIBILITIES:

  • Maintains current and thorough knowledge of incident reporting regulations (14NYCRR Part 624 and Part 625) as well as NYS Justice Center/OPWDD directives and guidance regarding reporting, investigation and monitoring of incidents.
  • Completes OPWDD 147 and initial IRMA entry when filing a reportable incident. Collects statements and documentation for preliminary investigation.
  • Completes thorough and quality investigations as assigned and meets regulatory and agency requirements.
  • Monitors incident follow-up in the electronic record for assigned investigations ensuring supporting documentation of corrective actions is adequate and submitted by the due date.
  • Uploads assigned incident investigations into IRMA.
  • Maintains investigation electronic folders.
  • Meets with supervisor as scheduled to review status of incident investigations, follow-up and performance.
  • Informs supervisor of any significant investigation concerns as they arise.
  • Completes Notable Event reviews as assigned. Ensures all follow up and corrective actions are addressed.
  • Provides training to employees in the areas of Incident Reporting and Abuse Recognition as assigned, or as needed.
  • Performs other duties relevant to the position as requested by the supervisor.
  • Advises direct supervisor of any obstacles to achieving responsibilities.

ESSENTIAL ACADEMIC AND EXPERIENCE REQUIREMENTS:

  • Associates Degree, Bachelor’s Degree preferred.
  • Knowledge of OPWDD Part 624 Regulations, preferred.
  • Previous experience in Quality Improvement/Compliance within OPWDD, OMH, DOH, OASAS, or OCFS preferred.
  • Successful completion of OPWDD/NYS Justice Center for the Protection of Vulnerable Persons Incident Investigation Training, upon hire.
  • Excellent verbal, listening and writing skills.
  • Strong typing skills and experience using Word.
  • Ability to work independently and motivate others.
  • Must be at least 18 years of age.
  • Reliable transportation (NYS Drivers’ Licensed required).
  • Maintain all required certifications/training by State regulations and CDS Monarch policy.

All experience and education requirements, except when required by federal, state, or local laws or requirements, may be waived at the discretion of management with the approval of the Program Vice President in collaboration with Human Resources

CORPORATE QUALIFICATIONS:

  • Adhere to all CDS Life Transitions and Monarch policies and procedures.
  • Attends mandatory Education and Training modules as scheduled, obtains, and maintains required certifications.
  • Act as a professional representative of CDS Life Transitions/CDS Monarch in regard to appearance, behavior, temperament, communication, language, and dress.

The listed salary range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting.
